This is a template, not a live message. Replace the placeholders and test the reply route. Avoid promising a completion time until you have tried the questionnaire yourself.
For appointment businesses, identify the completion record in your booking process. Coordinate the survey with your appointment reminder workflow so you can review the full message sequence.
How can a survey tool connect to other apps automatically?
Define the handoff before opening the connection settings. A trigger is the event that starts the sequence. An action is the job performed afterward. Write a plain instruction such as “When feedback arrives, add its rating and comment to our review queue.”

Should a poor rating trigger an automatic refund?

When should I add a reminder?
